Double Up with a MATCHING GIFT from your Employers

Double your donation with a matching gift from your employer.
To begin this process, first enter your employer's name and check to see whether they have a matching gift program. If they do, then follow the instructions provided on your company's online portal to fill out its matching gift form. Please also take a moment to fill out this separate form, scan it and email it to [email protected]. This will help us keep track of your matching donation request and we will send an acknowledgement when the gift is received. SMHSF is a 501 (c) (3) non-profit organization and therefore, all your donations are tax deductible. |
